Abstract: the ongoing National Quality Assurance Guidelines for Quality care in the DH/SDH /CHC and PHC there is a felt need for the same for the Sub Center. The Sub Center is spread like a network at the grass root level and ensuring Quality care service at this level will be a game changer and an ocean of change in the Health of our Nation. For this it was imperative to study what services the SC are giving today Äs is “What was envisaged in the IPHS and what is that the community wants. Following this how these services along with the outreach services of the SC be delivered in the best possible way keeping “Quality in mind/eyes” and able to satisfy the customer. The need was to measure this Quality because we know what gets measured gets improved with this in mind the Quality Score card is developed with exclusion and inclusion of the PHC score card. For this 4 Sub Centers of 2 District one Hill and one plain were chosen. The data from HMIS, RCH portal comparing with IPHS Guidelines and Handbook on enhancing performance of MPW was studied. SC were visited for observation of delivery of Services and Focus group discussions in the community and all the stake holders was conducted .SOP with the help of existing documents from SBA guidelines and Handbook on Skill training of ANM/SN was assembled . A Score card was finally developed and is being implemented in Sub centers in Pilot mode and within the next 06 months will be extended to entire State.
